GAMBARAN KADAR HORMON FT4 DAN TSH PADA ANAK SINDROM NEFROTIK DI RSUD ULIN BANJARMASIN

Abstract

Abstract:. One of the complications of nephrotic syndrome is hypothyroidism. Hypothyroidism in nephrotic syndrome is caused by glomerular filtration damage and proximal tubule dysfunction which causes the FT4 loss into urine. Hypothyroidism in children can cause delay in growth and disruption of the body's metabolism. The aim of this study was to describe the hormone levels of FT4 and TSH in children with nephrotic syndrome in relapsing phase and steroid-resistant nephrotic syndrome in Ulin General Hospital Banjarmasin in the period January 2017 - August 2018. The study sample consisted of 12 pediatric nephrotic syndrome patients who had met the inclusion criteria. The research is the descriptive research with a retrospective cross sectional approach. Result shown that of 12 patients consisting of 6 NS in relapsing phase and 6 SRNS, the overall FT4 levels were 20.31 ± 7.72, and TSH 4.5 ± 4.3 The mean FT4 levels in relapsing were 17.34 ± 3 and in SRNS 17.34, while TSH levels in relapsing  4.9 ± 5.3 and in SRNS 4.03 ± 3.42. This shows overall levels of FT4 and TSH normal, so it can be concluded that in children with relapsing and resistant nephrotic syndrome in Ulin General Hospital Banjarmasin don’t have hypothyroidism.

Abstrak: Salah satu komplikasi dari sindrom nefrotik adalah hipotiroid. Hipotiroid pada sindrom nefrotik disebabkan karena adanya kerusakan filtrasi glomerulus dan disfungsi tubulus proksimal yang menyebabkan FT4 terbuang lewat urin. Hipotiroid pada anak dapat menyebabkan keterlambatan tumbuh kembang dan gangguan metabolisme tubuh. Tujuan dari penelitian ini untuk mengetahui gambaran kadar hormon FT₄ dan TSH pada anak sindrom nefrotik relap dan sindrom nefrotik resisten steroid di RSUD Ulin Banjarmasin periode Januari 2017 – Agustus 2018. Sampel penelitian terdiri dari 12 orang pasien anak sindrom nefrotik yang telah memenuhi kriteria inklusi. Penelitian yang dilakukan bersifat deskriptif dengan pendekatan cross sectional  retrospektif. Hasil penelitian menunjukan dari 12 orang pasien SN, terdiri dari 6 orang SN relaps dan 6 orang SNRS secara keseluruhan rerata kadar FT4 20,31 ± 7,72, dan TSH 4,5 ± 4,3. Rerata kadar FT4 pada SN Relaps 17,34±3 dan pada SNRS 17,34, sedangkan kadar TSH pada SN Relaps 4,9± 5,3 dan pada SNRS 4,03± 3,42.  Hal ini menunjukan secara keseluruhan kadar FT4 dan TSH normal, sehingga dapat disimpulkan pada anak sindrom nefrotik relaps dan resisten di RSUD Ulin Banjarmasin tidak mengalami hipotiroid
