Abstract

Hospital food service units generate organic waste, primarily vegetable scraps from food preparation, which could be repurposed through sustainable resource management approaches. Conversely, hospital environmental sanitation practices continue to rely on chemical disinfectants, incurring recurring operational costs and potentially leaving chemical residues. This study aims to analyze the cost-effectiveness of an eco-enzyme produced from vegetable waste as an environmentally friendly alternative disinfectant in the Nutrition Unit of Ratu Zalecha Martapura Regional General Hospital (RSUD Ratu Zalecha Martapura). The study employed an experimental design complemented by a cost-effectiveness analysis. Effectiveness is determined by the ability of eco-enzymes and chemical disinfectants to reduce the Total Plate Count on floor surfaces, while costs are calculated from eco-enzyme production expenses, ready-to-use solution costs, coverage area, and estimated annual operational costs. The cost-effectiveness ratio is calculated by dividing the cost of the ready-to-use solution by the area covered. The results indicate that the eco-enzyme was comparably effective to the chemical disinfectant in reducing Total Plate Count; the Tukey HSD test showed no significant difference between them (p = 0.946). The cost of the ready-to-use eco-enzyme solution was Rp59/L, compared with Rp234/L for the chemical disinfectant. Assuming the same coverage rate of 12.5 m²/L, the cost-effectiveness ratio of the eco-enzyme was Rp4.72/m², compared with Rp18.72/m² for the chemical disinfectant. For the Nutrition Installation’s effective area of approximately 304 m², the estimated annual operational cost was Rp523,731.20 for the eco-enzyme and Rp2,076,171.20 for the chemical disinfectant, yielding potential annual savings of Rp1,552,440.00. Eco-enzymes derived from vegetable waste have potential as alternative disinfectants for non-clinical hospital areas, achieving equivalent sanitation efficacy at lower operational costs and supporting the principles of the circular economy.
